Financial Policy for Aquia Family Chiropractic and Acupuncture Center.

Payments Accepted

1. Cash.

2. Visa, MasterCard, American Express, Discover.

3. Personal checks, Money Order.

5. Health Insurance on assignment on approval

6. Health Savings Account (HSA)

We are happy to care for accident patient however, our policy is to Not Accept third-party insurance, assignment, Med-Pay, or PIP. We are sorry for any inconvenience. We will provide you with a Super Bill for any charges and payments to submit for reimbursement.

Payment is expected at time of service unless other arrangements are made in advance. Please contact our staff for the entire policy or with a specific question.

We will file your insurance claim at your request. A written referral from your primary care physician maybe required with certain HMO/PPO plans prior to your initial visit.

If you provide us with a copy of your insurance card we will attempt to verify your benefits. Remember, insurance benefits quoted by your insurance company may not be accurate and may change depending on whom you speak with and the phase of the moon. (humor)

Always request a written confirmation of your benefits and referral to be mailed or faxed.

There is a returned check fee of $25.00.

We realize that things come up and that you may miss a scheduled appointment, however, without a call to cancel that appointment deprives another patient the opportunity for care.

Communication about canceling your appointment is important and we ask for a 24-hour prior notice. You will receive a gentle reminder for the first occurrence, and a $25.00 missed appointment fee for the second. If you have missed a total of three appointments we reserve the right to deny another appointment for you.
